Moving Beyond "Root Cause"

This luncheon presentation offers .1 CEU

The quest for root cause or root causes may be leading us to a narrow and linear understanding of failure. Many times, events occur from the unusual combination of normal conditions. There may not actually be a “root cause” and it may not be a "path to failure."

At this luncheon presentation, you will learn how deep operational learning is yielding a more holistic view of failure. Also, the journey to failure may look a lot more like the journey to success then we may have realized. Shifting our conversation from “why” something happened to instead talk about “how” things happen is giving a more complete story. This may be a paradigm shift from our more traditional investigation fact finding approach to understanding failure!
